ADMISSION REQUIREMENTS

 

1.         A child must be 2-9 years old for the day or night program housed at 3103 Ruger

            Ave.  Children from birth to 2 are cared for at the Infant/Toddler Center housed

            at 2020 E. Milwaukee St.

 

2.         An application form must be completed and returned to Janesville Community

            Day Care Center, Inc. at least 3 business days before the intended day of

            enrollment.

 

3.         A deposit fee, the annual fee, as well as the first week’s tuition must be paid at the

            beginning of the first day of enrollment.  Families having more than one child

            should make arrangements with the director regarding payment of these fees.

 

4.         a.  Except for a school-age child, each child shall have an initial health examination

            not more than one year prior to nor later than 3 months after being admitted to the

            center, and a follow-up health examination at least once every 2 years after

            admission.  The health examination report shall be on a form provided by the

            department and shall be signed and dated by a physician, physician’s assistant, or

            HealthCheck provider.

            b.  The health examination requirement does not apply if the parent of a child

            requests in writing that the department grant an exemption based upon the

            parent’s adherence to religious belief in exclusive use of prayer or spiritual means

            for healing in accordance with the teachings of a bona fide religious sect or

            denomination.

 

5.         The immunization form must be signed and returned with the application and a

            health history.

 

6.         It is mandatory for you to make an appointment with the director to review center

            policies and to observe the center program.  Should you wish to visit the program

            thereafter, you need not make an appointment.

 

7.         A minimum of 2 days per week enrollment is required.

 

8.         The following supplies need to be given to day care personnel at the beginning of

            the first day of attendance:

a.      2 boxes of facial tissues

b.      1 package of napkins

c.      Paper Cups (5 oz.)

d.      Toothpaste with fluoride

e.      Toothbrush & toothbrush cap to cover bristles

f.       Extra set of clothing

            Those who are using diapers need also bring disposable diapers and baby wipes.
